Purpose of the Trip: As part of our Spring Break Camp program, these field trips are designed to provide educational and recreational experiences through exploring local businesses and participating in outdoor activities.
Assumption of Risk: I understand that participation in field trip activities involves certain risks, including but not limited to transportation, physical activity, outdoor elements, and visits to public spaces. I voluntarily assume all such risks on behalf of my child.
Medical Consent: In the event of a medical emergency, I authorize camp staff to seek necessary medical treatment for my child if I cannot be reached. I accept responsibility for any medical expenses incurred.
Behavioral Expectations: I understand that my child is expected to follow all rules, listen to staff instructions, and behave respectfully. Failure to do so may result in my child being removed from future field trip activities.
Release of Liability: I release hold on Revolution Martial Arts Langley (0872122 BC Ltd.) and it’s staff, volunteers, and affiliates from any and all claims, demands, or causes of action arising from my child’s participation in these field trips.
Photo/Video Consent:
I grant permission for my child’s photo or video to be taken during field trip activities and used for promotional purposes.
